Self-Certification

 

I certify the information provided to the PPE Unite Program is true and correct. Additionally, I confirm the following is true: 

1. My business is located and does business within California. 

2. I operate either as a sole proprietor, partnership, LLC, C or S-Corp, micro-enterprise or nonprofit.

3. The PPE my business receives via PPE Unite is for my workplace employee safety.

4. I will abide by the rules of the membership plan that I subscribed to and stay current with my monthly dues.

5. I acknowledge that the monthly subscription fee for my membership is not used for the purchase of the PPE supplies and that it is used to support PPE Unite staff and operations.

 

Liability Product Use Release  

 

Products provided via the PPE Unite program and on PPEUnite.org are primarily intended to support the Small Business Sector and Employee Workplace Safety. By receiving the products offered and made available through this website, you explicitly release PPE Unite and its partners from all liability and waive legal rights related to use of the product. PPE Unite does not guarantee the quality, integrity, condition, usability, or the efficacy of the products. This shall be the case no matter how or where the item was used. 

 

By registering or receiving product from the PPE Unite program, you agree to:

1. Waive, release, and or/make any claims against PPE Unite, Logisticom or any other owner or operator of businesses offering products/services through this website, or any one of the aforementioned entities and each of their insurance carriers, subsidiaries, affiliates, officers, directors, shareholders, members, representatives, assignees, employees, volunteers and agents, as well as any manufacturers and distributors (hereinafter the “Indemnified Parties” collectively, the “Indemnified Party” individually) that are based on, arise or result from, in whole or in part, use of the product. 

2. Indemnify, defend, and hold the Indemnified Parties harmless, from any and all claims, demands, actions, causes of action, losses or liabilities whatsoever arising from or related to participation in any activity and any loss, damage or injury, including death, that may be sustained by user or assignee, or caused to others or their property by user or assignee.  User also agrees to pay all costs, including reasonable attorneys’ fees and disbursements, incurred by any Indemnified Party in defending an investigation, claim or suit brought by or on behalf of the user or assignee.

3. Forego rights as a "buyer", "purchaser", or "titleholder" of the products received since you are not purchasing the products. Your subscription fee is a service fee and does not constitute or imply a purchase of the products. You understand that the products are provided to you for free and you will not receive a purchase contract, bill of sale, receipt, nor will you be entitled to refunds, credits, guarantees, warranties, claims, remedies afforded to a buyer or purchaser.
